ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-19Rn-944 <br />INSPECTION DATE: > i L2AinR INSPECTORS INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 11/26/08. The main purpose <br />of the inspection trip was to look at the recently competed drainage enhancement of the <br />Phase IVC perimeter ditch area, which had experienced a few problems in the past. <br />Present during the inspection was Mr. Mark Vanoni of CC&V. As approved under Technical <br />Revision 52, the perimeter ditch had been enhanced to handle the probable solution flow <br />that will report to the area. DRMS had conditionally approved TR-52.Since full application <br />of solution commenced in the area, 14 days ago, the DRMS wanted to verify and walk the <br />ditch and nuke sure the ditch was handling the flow. The length of the ditch was walked <br />and inspected, according to the pad operators, approximately 1000gpm, was being applied to <br />the pad at the time of the inspection. The ditch appeared to handle the flow and no <br />problem was noted. <br />Inspected the phase I,II,III&IV LECAH pad areas and recorded the following readings from <br />the high and low volume solution collection transducers. <br />Phase I high Volume solution collection transducer # 1 was recorded at 46.5 (Transducer <br />read out meter was replaced three weeks ago, it was not functioning for quite a while as <br />reported in the past inspections. # 2 was at 62.5 and # 3 was at 51.4. Piezometer for the <br />pond was at 57.5 feet. Low volume transducer # 1 was recorded at.38 and # 2 was at.34. <br />Phase II High Volume solution collection transducer 44 was recorded at 38.8 # 5 was at <br />28.6 and # 6 was at 38.6. Low volume solution collection transducer # 1 was recorded at.17 <br />and # 2 was at .61. Piezomter for the pond was at 39.9 feet. <br />Phase IV high volume solution collection transducer # 7 was recorded at 43.5 ,# 8 was at <br />43.4 and # 9 was at 44.3 and # 10 was at 44.2 . Low volume transducer # 1 was recorded <br />at13.4 and # 2 was at 15.0 inches of hydrostatic head. <br />Inspected the external pond, it was holding what appeared to be a partially frozen water. <br />Low volume solution collection transducers for the pond were measured at 13.8 and # 2 was <br />at 12.3. <br />„ <br />..k <br />I & E Contact Address <br />NAME: MR.
