The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when considered <br />by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />During the inspection, it was discovered that there was no sign posted at the entrance to 12/ 30708 <br />the site. This is a problem related to Section 3 1 12(1) of the Construction Materials Rules <br />and Regulations for failure post a sign which is clearly visible from the access road with the <br />name of the Operator, a statement that a reclamation permit for the operation has been <br />issued by the Colorado Mined Land Reclamation Board; and the permit number. <br />PB or PV: PR <br />TOPIC(S): Sm <br />Corrective Action: Place the appropriate sign at the access point to the site within thirty 130) <br />days of receipt of this inspection report and send photos to DMG for verification <br />2' 12/30/08 <br />During the records review conducted relative to this inspection it was discovered that the <br />mine has been inactive in excess of 180 days without the operator filing for temporary <br />cessation. The is a problem related to Rule 1.13.5 of the Construction Materials Rules and <br />Regulations for failure to file a Notice of Temporary Cessation in writing to the Office. <br />PB or PV: PB_ <br />TOPIC(S): AR <br />Corrective Action : Either file for temporary cessation of the site according to the <br />Construction Materials Rules and Regulations transfer the permit to an entity that will <br />conduct mining at the site, or submit an annual report that reflects some type of mining or <br />reclamation activity will occur over the course of 2009, within thirty (30) days of receipt of this <br />inspection report
