Vernon, M.D. <br /> Executive Director <br /> PURSUANT <br /> to the provisions of the Colorado Solid Wastes Disposal Sites and Facilities <br /> Act, C.R.S. 1973, 30-20-102, and the Colorado Domestic Sewage Sludge <br /> Regulations, 5 CCR 1003-7, this Notice of Authorization to Apply Domestic <br /> Sewage Sludge certifies the Summit County, Snake River Wastewater Treatment <br /> Plant as meeting all applicable regulations of the Colorado Department of <br /> Health and authorizes the aforementioned to land apply treated sewage sludge <br /> for beneficial purposes to the property described below: <br /> Climax: Mine/Mayflower Tailing Deposition Area; Colorado Department of Health, <br /> Municipal Sludge Management Program # 0405; portions of Sections 13, 14 and <br /> 24, Township 7 South, Range 79 West; Summit County; as shown on attachment. <br /> APPLICATION <br /> shall be performed in accordance with applicable criteria of the Colorado <br /> Domestic Sewage Sludge Regulations, 5 CCR 1003-7 (as amended) unless the <br /> Department has issued a variance per the requirements of 5 CCR 1003-7, Section <br /> 1.D. <br /> SPECIAL CONDITIONS <br /> if required are attached to assure compliance with applicable criteria of the <br /> Colorado Domestic Sewage Sludge Regulations, 5 CCR 1003-7 (as amended) . <br /> SELF MONITORING REPORTS <br /> shall be submitted to the Colorado Department of Health per the requirements <br /> of the Colorado Domestic Sewage Sludge Regulations, 5 CCR 1003-7, Section 14.G. <br /> FEE PAYMENT <br /> in the amount of two dollars per dry ton of sludge which is land applied to <br /> the site prior to July 1, 1990 and two dollars and forty cents per dry ton for <br /> sludge applied on or after July 1, 1990, shall be made to the Colorado <br /> Department of Health per the requirements of the Colorado Domestic Sewage <br /> Sludge Regulations, 5 CCR 1003-7, Section 15.B. <br />
