(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1985-008 <br />INSPECTION DATE: 10/ 3/08 INSPECTOR'S INITIALS KAP <br />OBSERVATIONS <br />1. This inspection was performed as part of the Division of Reclamation, Mining, and Safety's four-year monitoring <br />program. Present at the inspection was Kate Pickford, DRMS. Weather at the time of the inspection was clear, <br />windy, and approximately 35 degrees. <br />2. The site is accessed off of CR 543 approximately'/ mile south of its poorly marked intersection with Hwy 69, <br />located on the east edge of Gardner. The access road is shared with another operation, the Orlando pit, operated <br />by Huerfano County. The Orlando Pit is located to the south of the Leone-Gardner Gravel site. A sign is posted at <br />the entrance and the activity at the site appears to be contained within the permit boundaries. <br />3. Disturbance at the site consists of a square excavation and a large stockpile of material. Groundwater is exposed <br />as a result of the excavation activity. The Division finds no evidence that there is a well permit or augmentation <br />plan associated with the site (see page 3). <br />4. The pit slopes are fairly steep and will need to be graded down to no steeper than 3V:1 H for reclamation. <br />5. The current topsoil stockpile has very little vegetation (see page 3). Only a small pile of topsoil appears to be <br />available for reclamation, and does not appear to be adequate for the 6 inch layer called for in the reclamation <br />plan. The Operator will need to address how he intends to supply the additional topsoil needed. Otherwise, the <br />Division will need to additionally increase the financial warranty for the site to account for the importation of topsoil <br />(see page 4). <br />6. A new financial warranty in the amount of $49,660.00 has been calculated for the site, based on current <br />conditions. The financial warranty may be further increased, based on responses submitted and corrective actions <br />taken by the Operator.
