(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1977-378 <br />INSPECTION DATE 10/8/08 INSPECTOR'S INITIALS WHE <br />OBSERVATIONS <br />This inspection occurred as part of the Division's routine monitoring of permitted sites. The Sunnyside <br />Mine permit includes lands at the American Tunnel, Mayflower Tailings Pond No. 4, Terry Tunnel, and <br />Lake Emma area, totaling 148.93 acres. The Operator has commenced final reclamation at all areas of <br />the permit. Affected lands will be reclaimed to support wildlife habitat and rangeland post-mining land <br />use. The Division holds $60,534 financial warranty. Six photos accompany this report. <br />Lake Emma: The Operator originally backfilled the subsidence feature at Lake Emma during 1992. <br />Since that time additional backfill material has been applied periodically as the backfilled material settled. <br />Significant settling of the backfill material appears to have ceased and the Operator has not added <br />additional backfill in recent years. The bottom of the backfilled basin has been graded to provide positive <br />drainage to the south. Evidence of slumping, settling or excessive erosion was not observed. <br />Terry Tunnel: The reclaimed mine bench at the Terry Tunnel has been seeded repeatedly. The most <br />recent seeding occurred last fall, following the re-scarification of the entire mine bench. Vegetative cover <br />was establishing in the bottom of the grooves resulting from the scarification. The mine bench appeared <br />stable, evidence of slumping, settling or excessive erosion was not observed. A seep or spring is located <br />nearby the backfilled portal and may be associated with a larger seep/spring observed in a road cut <br />upslope of the backfilled portal. <br />Mayflower Tailings Pond No. 4: Reclamation for the Mayflower Pond 4 appeared near complete. <br />Vegetative cover appeared at least equal in extent of cover to the natural vegetation in surrounding <br />areas, with the exception of a small area near the center of the pond. A small stockpile of reclamation <br />materials remains at the south end of the pond. Remaining reclamation to be completed appears limited <br />to removal of all mining related equipment and stockpiled reclamation materials. <br />American Tunnel: Vegetative cover for the portions of the Herbert Placer previously occupied by the <br />water treatment ponds was well established. Portions of the Quonset but located on the Herbert Placer <br />had been damaged by snow loading.
