(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1986-109 <br />INSPECTION DATE: 10/10.408 INSPECTOR, S INITIALS: KAP <br />OBSERVATIONS <br />1. This inspection was conducted as part of the Division of Reclamation, Mining, and Safety's 4-year <br />monitoring program. Present at the inspection was Kate Pickford, DBMS. Weather at the time of the <br />inspection was cloudy, drizzle, and approximately 55 degrees. <br />2. The access gate is locked. A sign is posted at the entrance and the boundaries are easily identified at <br />the site, consisting of a cable fence and the steep embankment where the underground accesses are <br />located. The site is well maintained and stable. The only areas that could present a problem are the <br />two pinnacles of limestone along the edge of the site. During reclamation, these features will need to <br />be removed or reinforced to prevent them from falling down slope. <br />3. Activities at the site appear to be within the guidelines of the approved permit. A separate bond <br />calculation will be conducted to determine whether the current bond amount of $12,000.00 is <br />adequate for the remainder of reclamation at the site.
