Preliminary <br /> review of the information received determined that the following items must be received before the Division can <br /> consider your application as being submitted and technical review can begin: <br /> APPLICATION FORM Responsibilities as a Permittee: Pages 4 and 5: The person initialling this <br /> section of the application form must be an officer of the company. Also, unless Climax Molybdenum <br /> Company is a Joint Venture or Partnership item No. 10 should not be initialed. Please have this section <br /> redone by an officer of the company. The Division cannot accept the initials of the Mill Manager on Pages <br /> 4 and 5. <br /> Signature: Page 6: The Division cannot accept the signature of the Mill Manager on the application. <br /> Please have an officer of the company sign the application on the signature line and list their title on the <br /> Title line. Also, the company's Corporate Secretary must sign and affix the corporate seal as indicated <br /> by the highlighted area. <br /> EIIIIIBIT O Proof of Mailing of Notices to the Soil Conservation District (Rule 6.4.17): Please <br /> submit proof of actual delivery or proof of mailing by Certified Mail, return receipt requested, of Notice <br /> of the Application to the local Board of Soil Conservation Districts (SCD), Board of Supervisors. <br /> Specifically, you must notify for Eagle County - Eagle County SCD; for Summit County - Middle Park <br /> SCD; and for Lake County - Lake County SCD. <br /> Your application will not be considered submitted until the information listed above is received and found sufficient <br /> to begin our review. A decision date will be established 90 days from the date of receipt of all of the requested <br /> information. Additionally, if you have already published notice you will need to republish notice, after the <br /> Division considers the application submitted. This notice must be published once a week for four (4) consecutive <br /> weeks, starting within ten(10)days of the date your application is considered submitted. We will notify you when <br /> you should initiate republication of your notice. The final date for receiving comments is the 20th day after the <br /> fourth publication or the next regular business day. <br />
