(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-2006-039 <br />INSPECTION DATE: 10/7/2008 <br />INSPECTOR'S INITIALS THM <br />OBSERVATIONS <br />This inspection was conducted as part of the normal program established by the Colorado Division of Reclamation, <br />Mining and Safety. The North Clear Creek Construction Materials Mine is a 110c permitted site located <br />approximately 15 miles north of Debeque, Colorado on Clear Creek Road. Julie Justus of Chevron North America <br />Exploration and Production was present during the inspection. <br />The mine identification sign and permit boundary markers were in place and in compliance with rule 3.1.12. The <br />sign is located at the main entrance to the gravel mine. Permit boundary markers were comprised of fence posts that <br />were evenly spaced and easily identified along the perimeter permit boundary. <br />Various pieces of heavy equipment were located within the permit area. A bulldozer was observed working the <br />south end of the mine face. A few small stockpiles of construction material were observed at the base of the active <br />mine face. A topsoil stockpile was observed along the northern boundary of the permit area. The permit area <br />appeared to have approximately 60 percent of the material removed.
