(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1928-0-57 <br />INSPECTION DATE: 9/11 /m INSPECTOR, S INITIALS: KAP <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Reclamation, Mining, and Safety's 4-year <br />monitoring program. Present at the inspection were Kate Pickford, DRMS and Robert Scott, Canyon <br />Sand and Gravel. Weather at the time of the inspection was clear, calm, and approximately 65 <br />degrees. <br />2. The site is accessed from CR 146 west of Pagosa Springs off of Hwy 160. Snow Ranch Road branches to <br />the left from CR 146, and the site is located at the end of the this road in the National Forest. A sign is <br />posted at the entrance to the site. Mining at the site is nearly complete. The site currently consists of <br />long, narrow pond with a peninsula jutting into it. The peninsula contains stockpiled material along its <br />perimeter which will be graded during the final reclamation of this area. <br />3. The pond itself possessed banks that are steeper than 3:1. The only bank that was mined, however, <br />located along the north western pond edge, is graded to 3:1. Therefore, the steeper pond slopes are <br />allowable for final reclamation. The steeper slopes have been reinforced with large boulders which <br />serve to stabilize these slopes and allow people and wildlife to exit the pond despite the steep slopes. <br />The northern bank of the pond is near vertical, as the operator has installed a kiva-like structure. Stairs <br />were installed in the vicinity of the kiva which also allow for safe exit from the pond. <br />4. A crusher and processing area are located west of the pond. There are multiple above ground storage <br />tanks in this area that contain fuel for operations. At the time of the inspection, these tanks had no <br />secondary containment. The operator was advised to install secondary containment and he stated he <br />would probably opt to use stock tanks to place the storage tanks into for the containment. <br />5. The Office of the State Engineer confirmed that they have a well permit and augmentation plan for the <br />exposed groundwater at the site, 2004 number 61272F. <br />6. Activities at the site appear to be within the guidelines of the approved permit. A separate bond <br />calculation will be conducted to determine whether the current bond amount of $1,974.00 is adequate <br />for the remainder of reclamation at the site.
