ATTACHMENT TO ANNUAL REPORT <br />VAN 4 MINE <br />PERMIT NO. M-1997-032 <br />OCTOBER 30, 2008 <br />Maintenance and site clean up work were conducted at the Van 4 Mine site in August of 2008. <br />No mining occurred at the Van 4 in 2008; however, Denison is currently evaluating the Van 4 <br />Mine and anticipates that mining will continue at the Van 4 within the upcoming year and that <br />approximately 12,000 tons of ore could be produced in 2009. Prior to initiating mining <br />activities, the surface facilities at the Van 4 will need to be rehabilitated; however, the existing <br />Division permitted boundary will not be changed. Prior to initiating mining activities, the <br />Division will be formally notified of any site disturbance activities at the Van 4 Mine. <br />In addition, a vent hole was reopened in May of 2007; however, it was determined that the hole <br />was not useable and it was resealed and reclaimed. Vegetation status of the vent hole was <br />evaluated in July of 2008 and vegetation was not sufficiently established to propose bond <br />release. Reseeding has been completed in the area this fall and a visual inspection will occur in <br />the spring to determine vegetation status. <br />In March of this year, the Division provided notification to Denison Mines that the Carnation <br />Mine is a Designated Mining Operation. Denison has initiated characterization of the waste rock <br />and ore. Potential impacts to groundwater and surface water from exposure of the waste rock is <br />ongoing. In July and September, work plans for waste rock and groundwater analysis were <br />provided and approved by the Division. In August of this year, Denison conducted waste rock <br />characterization sampling at the Van 4. <br />An Environmental Protection Plan will be prepared for the Van 4 and provided to the Division in <br />March 2008. This plan will include the results of the material characterization, drainage report, <br />and surface water evaluations (no groundwater is present in the Van 4). <br />No changes occurred in the Permit M-1977-032 area during 2008, and the site map, submitted <br />with the Permit application, on file with the Division, remains current. Stormwater evaluations <br />
