Outfall <br />005A/005X/MON005 flowed the last three weeks of September. No permit exceedances were detected, however <br />note that one parameter (mercury) is omitted from the MON5 DMR due to an oversight in filling out the chain of <br />custody. Please note that even though most sites did not discharge in September, some of these sites <br />(i.e. 002A and 006A) did flow earlier during the quarter and do require quarterly DMR reporting for a few <br />of parameters (e.g. conductivity & TDS). Thus, the enclosed reports contain this quarterly information, <br />while the monthly reporting parameters have been left blank since they did not flow in September. <br />A Quarterly Whole Effluent Toxicity (WET) test was collected for analysis for Outfall 005X during the third quarter. The test <br />indicated that the effluent were not acutely toxic to Daphnia magna survival (see attached laboratory report). <br />In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover <br />the outfalls, which did not discharge in September 2008. This includes Outfalls 003A (Pond B), 004A (Pond C), and <br />007A/007X/MON 7 (Pond F), and 008A (Pond G) <br />CERTIFICATION <br />I certify under penalty of law that I have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, l believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties for submitting false information, including the <br />possibility of fine and imprisonment.
