(Page 3) <br />MINE ID # OR PROSPECTING ID # M-1991-054 <br />INSPECTION DATE 9/9/08 <br />INSPECTOR'S INITIALS RCO <br />The operator is reminded that the permit includes 1.9 acres, comprised of two separate areas (upper and lower dumps). The <br />slopes under each dump are not included in the permit. All disturbance, either deliberate or inadventent, outside the boundary <br />maybe a possible violation and must be avoided. If additional land is needed for future operations, the operator may apply for <br />a permit conversion. <br />The annual reports do not reflect that operations have begun again at the site. The operator must describe all activities in <br />future annual reports. They should contain information on all activities performed: mining, reclamation, earthwork, <br />maintenance, weed control, etc. Since there are new areas of disturbance, the annual reports must also include an accurate <br />map of the site and the activities. The annual reports must also include current contact information, since it is not completely <br />correct (phone number is disconnected). <br />This is a 110(1) permit, whose bond maximum is capped by law at $1500. There is a $1500 bond posted for the permit. <br />Current reclamation costs exceed that amount, but this is not a problem. <br />No reclamation has been performed at the site. <br />No further items were observed during the inspection.
