(Chevron) has contracted- WWC, Engineering (WWC) to assist in attaining bond release for <br />the former Edna Mine, Permit Number C-80-001, located in Routt County, Colorado. Through prior verbal <br />communication you have granted permission to transfer ownership rights for well(s) to your name. The <br />following well(s) located on your property are to be transferred to your name: <br />0 27086 <br />• 46214 <br />• 88538 <br />The following materials are provided under this cover letter: <br />• Form No. GWS-11. Change In Ownership/Address <br />WWC is transmitting this package requesting your signature on the GWS-11 form as New Owner. After the <br />form(s) has been signed please send them back to WWC in the enclosed envelope. WWC will then forward the <br />package to the State of Colorado, Office of the State Engineer to commence Change in Ownership/Address of the <br />well(s).
