(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1997-039 <br />INSPECTION DATE 09/24/08 INSPECTOR'S INITIALS PSH <br />OBSERVATIONS <br />The Williams/Illinois Gravel Pit, File No. M-1997-039, was inspected by Peter Hays with <br />the Division of Reclamation, Mining and Safety (Division) as part of the Division's 4-year <br />monitoring inspection program. A representative from Wyatt Redi-Mix Co was not on site <br />during the inspection. <br />The Williams/Illinois Gravel Pit is located approximately 3 miles northwest of Rand, CO on <br />Hwy 125. The site is permitted for 9.9 acres and was previously inspected by the Division <br />on April 27, 2004. <br />The recent annual reports indicate mining activities are occurring in Stage 2 (west side) <br />of the pit and Stage 1 (east side) of the pit is being used for equipment storage and <br />processed material stockpiles, which is consistent with the observations made during the <br />inspection. <br />The job trailer, crusher and conveyors are located in the middle of the pit. The active <br />highwall is approximately 8 feet high and approximately 125 feet east of the west permit <br />boundary. Eight inches to 1 foot of topsoil has been removed in advance of mining to the <br />west permit boundary, which is consistent with the approved reclamation plan. Well <br />vegetated topsoil and overburden stockpiles were located along the north, east and west <br />permit boundaries. No erosion was observed. The eastern and north slopes were <br />approximately 16 feet deep and reclaimed to a 4:1 slope. <br />Mr. Richard Wyatt indicated Wyatt Redi-Mix Cc intends to convert the Williams/Illinois <br />Gravel Pit to an 112c Construction Materials Reclamation Permit in the near future and <br />requested information from the Division. A copy of Rule 1.11 - Conversions and an <br />application form are attached. Permit conversion information and forms can also be <br />obtained from the Division's web page at http://mining.state.co.us. <br />The Division currently holds a financial warranty of $7,500. The financial warranty will <br />be recalculated by the Division during the 110c to 112c permit conversion process. <br />An entrance sign was posted and permit boundary markers were observed as required by Rule <br />3.1.12. No problems were noted during the inspection. <br />The permit area, haul road, entrance sign and trailer/conveyor locations were GPS surveyed <br />with a Trimble Geo XT by the Division.
