MINE ID # OR PROSPECTING ID #: m-gn0A-n2A <br />INSPECTION DATE: in/117.1gnnA INSPECTOR=S INITIALS: IIF <br />OBSERVATIONS <br />This was a normal monitoring inspection of the Leone Gravel Pit; permit number M-2004-026. The site is <br />located about six miles north east of the town of Trinidad, Colorado in Las Animas County. i, Jared Ebert of <br />the Colorado Division of Reclamation, Mining and safety conducted the inspection. Mr. Frank i.cone of Leone <br />Sand and Gravel, LLC. accompanied me on the inspection. <br />The permit area for this site is 56 acres. A road leads from I Iighway 350 south into the permit area. Prior to <br />issuance ofthis permit, an excavation existed in the south east portion of the site. At this time, this area is <br />stable and flat, there is a stockpile of topsoil located in this area. The rest of the site was to be mined in four <br />phases. A small intennittent creek nuns through the permit area. Phases 1 and 4 are located east ofthe creek <br />and Phases 2 and 3 are on the west side of the creek. At the time this site was permitted, one CIG (Colorado <br />Interstate Gaslinc) gas line ran through the mine site. Since then, CIG has installed two other lines within the <br />permit area. According to Mr. Leone, this has limited the amount of material they have been able to mine. The <br />operator appears to be maintaining the proper set-backs from the various gas lines. <br />At this time, there is a small excavation taking place on the very southern boarder of Phase 1. The majority of <br />Phase 1 has been back filled and graded to a 3:1 slope. Topsoil had recently been spread on part of the slope <br />found in Phase 1. Several other portions of Phase i need to be backtilled and sloped. Phase 4 has been mined <br />and backfilled and sloped to 3:1 or less, the areas is densely covered with Russian thistle and other (orbs. It <br />may be necessary to remove these weeds to help the desired vegetation to become established. <br />According to Mr. Leone, they did not find much gravel in Phase 2 or 3 so they will not be excavating further in <br />that area. A small excavation was noticed on the west side of the creek in either Phase 2 or 3. Mr. Leone stated <br />that they had removed topsoil from that area, and that is would be re-spread and reclaimed. <br />This operation appears to be in compliance with the approved mine plan. <br /> <br />the west of the creek in Phasc 2 or 3. <br /> <br />exca\ ation to <br />Figure 2.)
