WHIRLWIND MINE <br />WELL CONSTRUCTION AND TEST REPORT (FORM NO. GWS-31) <br />SUPPLEMENTAL INFORMATION SHEET <br />Well permit number 66419 applies to an underground mine sump, not an actual water well. As such, <br />much of the information requested in Forms GWS-31 is not applicable to our mine sump. The following <br />supplemental information is intended to clarify areas where information is not applicable and provide <br />additional information that is applicable. <br />The mine sump was constructed by a previous mine operator in the early 1980's. Rehabilitation of the <br />Whirlwind Mine sump was completed by Energy Fuels Resources in early July 2008. Rehabilitation <br />included mucking out the accumulated sediment, stabilization of the sump, and construction of a sump <br />filter. Several feet of sediment was mucked out that had accumulated in the sump during previous <br />mining operations and while the mine was inactive. Stabilization of the sump walls and roof consisted <br />bolting on chain-link fencing followed by shot-creting. The sump filter consists of two large culvert <br />pipes, oriented vertically with one standing inside the other. Holes were cut in the culverts and gravel <br />was placed between them. The mine pump sits inside the culverts, suspended from a rolling chain hoist. <br />In addition, a catwalk is located along the wall of the sump to provide access to the sump filter and <br />pump. The entire sump measures approximately 17 feet wide by 46 feet long and ranges from 15 to 20 <br />feet high. <br />The following information provides clarifications to Form GWS-31 on an item by item basis: <br />Item #4: Ground surface elevation is at the mine portal. The date of construction is unknown as it was <br />performed by a previous mine operator in the early 1980's. <br />Item #'s 5, 6, 7, 8, 9, 10 and 12: Not applicable as the mine sump is not a drilled well. <br />Item #11: No disinfection is required as the water is being pumped from the mine for mine dewatering <br />purposes and is not used as drinking or potable water. The mine water is treated to standards set forth <br />in our CDPS Permit (No. CO-0047562) prior to being discharged to the surface into the middle tributary <br />of Lumsden Creek. <br />Item #13: Construction company, address, and license # is unknown as it was performed by a previous <br />mine operator in the early 1980's and not applicable as this is not a well, but instead a mine sump.
