Attached <br />with each form is a supplemental information sheet to clarify information provided on the <br />form. The Whirlwind Mine is located at 30100 5110 Road near Gateway, Colorado. <br />Energy Fuels has reopened the Whirlwind Mine and intermittent dewatering of the <br />existing and future mine workings is required to conduct mining operations. <br />Groundwater is not being pumped from an actual water well, but rather is allowed to <br />collect in a sump located in the mine workings. The underground workings and sump <br />were constructed in the early 1980's during previous mining efforts in the Whirlwind <br />Mine. Energy Fuels has rehabilitated this sump, including mucking it out, installing a <br />filter, and installing a new pump. <br />Water seeping into the mine sump is of poor quality and not suitable as potable water. <br />This water is used for drilling inside the mine and brought out of the mine as moisture in <br />the waste rock and ore. Excess water, if any, is pumped to the surface, treated, and <br />discharged to the surface drainage under a CDPS permit.
