6.4.12 EXHIBIT L - Reclamation Costs
The Division will calculate the bond requirement for the site based on information in the
application as soon as assumptions in Exhibit D are confirmed.
6.4.13 EXHIBIT M - Other Permits and Licenses
This section is adequate as submitted.
6.4.14 EXHIBIT N - Source of Legal Right to Enter
This section is adequate as submitted.
6.4.15 EXHIBIT O - Owner(s) of Record of Affected Land (Surface Area) and
Owners of Substance to be Mined
This section is adequate as submitted.
6.4.16 EXHIBIT P - Municipalities Within Two Miles
This section is adequate as submitted.
6.4.17 EXHIBIT Q - Proof of Mailing of Notices to County Commissioners and Soil
Conservation District
This section is adequate as submitted.
6.4.18 EXHIBIT R - Proof of Filing with County Clerk and Recorder
This section is adequate as submitted.
6.4.19 EXHIBIT S - Permanent Man-Made Structures
The mining plan shows that you are creating a new access road to the mine area from CR
FF. Because this new road is also considered part of the affected area of the mine, and is
within 200 feet of the county road, you should have an agreement in place with the
county for how any damage to County Road FF will be addressed. If this is already a
part of the special use agreement with the county (if it includes access to the road, and
how any maintenance or damage to the road will be addressed) you may provide a copy
of that instead. The Division understands and accepts that you will not mine within 200
feet of any other permanent man-made structures including fences and utilities. Please
make sure to delineate the boundary of the mining area appropriately and clearly, and
keep all mining activities within that area, to avoid permit violations.
