MINE ID # OR PROSPECTING ID #: M-1989-142 <br />INSPECTION DATE: 9/96..19008 INSPECTOR=S INITIALS: JI F <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when <br />considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be <br />notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br /> Problem Possible Violation: November <br />1.) At several locations along the pit slopes of the north east lake and the south lake <br />erosion th <br /> , <br />gullies and ruts have formed. This is a problem at this time for failure to protect the <br />affected land from erosion pursuant to C.R.S. 34-32.5-116 (4) (j). 30 Zoos <br />Type PB <br />Topic SC <br />Corrective Action: <br />Please provide photo documentation to the Division that the various erosion gullies and <br />ruts have been controlled and that the pit slopes have been reconstructed and <br />stabilized. Please submit this documentation by November 30th, 2008. <br /> <br />"2.) Problem Possible Violation: <br />On the western pit slope of the south lake, the slope was found to be steeper than a 3:1 November <br />t'' <br /> <br />horizontal to vertical ratio as it is specified in the approved reclamation plan. Please submit a letter of notice to the Division when the pit slope has been <br />constructed and the Division will conduct a follow-up inspection for the corrective action.
