ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1980-944 <br />INSPECTION DATE: 9.416.40.8 INSPECTORS INITIALS: R_ <br />OBSERVATIONS <br />The Division conducted a monthly monitoring inspection of the site on 9/15/08. Present <br />during the inspection was Mr. Mark Vanoni of CC&V. <br />Inspected the Valley Leach Phases I thru IV. Ore was being loaded along the Phase IV PAD <br />AREA at the time of the inspection. Some lift remedial work was being conducted along the <br />Phase II leach pad area. <br />While in the area, collected the following readings from the Phase I,II,III and IV High <br />and Low volume solution collection transducers for the pregnant ponds. <br />Phase I High Volume Solution Collection Transducer # 1 read out meter was not functioning. <br /># 2 was at 41.4 # 3 was at 43.2.Piezometer for the pond was at 53.8 feet. Low Volume # 1 <br />was at .39 and # 2 was at .35 <br />Phase II & III High Volume Solution Collection transducer # 4 was recorded at 41.7 # 5 was <br />at 39.9 and # 6 was at 38.6. Low volume solution collection transducer # 1 was at .17 and <br /># 2 was at .59.Piezomter for the pond was at 43.1 feet. <br />Phase IV High Volume solution collection transducer # 7 was recorded at 48.1 # 8 was at <br />48.0 and # 9 was at 49.5 and the pipe or # 10 was at 48.4. Low volume solution collection <br />transducer # 1 was at15.4 and # 2 was at 9.4 inches of hydrostatic head. <br />Inspected the External pond, it was holding some water. Low volume solution collection <br />transducer # 1 was at 15.4 and # 2 was at 9.4 inches of hydrostatic head( All other <br />hydrostatic head measurements are in feet of hydrostatic head unless otherwise noted.) <br />Inspected the under drain area, all the lower ponds were almost dry, the southwestern side <br />of the large dissipation basin was holding some water from the recent storm, but was in <br />good repair. Both the South under drain and 4 inch pipes were discharging at the time of <br />the inspection. Inspected AG 1.5 it was dry. <br />Inspected the crusher area, it was active at the time of the inspection. Most of the storm <br />water diversion sumps for the haul roads were holding some water. CC&V is going to conduct <br />additional routine maintenance before the winter season, according to Mark. <br />Inspected the Cresson pits, both the main and East Cresson (Wild horse area)were active. <br />East Cresson backfill was continuing. Based on the current progress, it will not be too <br />long before the entire pit is backfilled as approved in the TR and Amendment #8. <br />Inspected the Victory pad area. All the surface water diversion ponds were found to be in <br />good repair. Large surface water diversion pond was dry. The sloped leach pads that will <br />under go reclamation soon were in good repair, no erosion features were noted. <br />Inspected the are where additional GW monitoring wells were being drill to cover the <br />Grassy Valley up gradient area. Case advancing drills were being used and drilling was <br />continuing at the time of the inspection. <br />At the request of CC&V, inspected the old ore sorting house that is located in the North <br />Cresson MLE area known as the Sims ore house.CC&V wanted DRMS to document the condition of <br />the structure and the difficulty of moving it in one piece. Pictures were taken and <br />provided to Miss Susan Collins and Mr. Dan Corson of SHPO.
