MINE ID # OR PROSPECTING ID #: M-1923-175 <br />INSPECTION DATE: _ n9.19A/n7 INSPECTORS INITIALS: KAP <br />OBSERVATIONS <br />1. This inspection was performed as part of the Division of Reclamation, Mining, and Safety's four-year <br />monitoring program. Present at the inspection were Kate Pickford, DRMS, and Ron Barbosa, Southwest <br />Readi-mix, Inc. Weather at the time of the inspection was clear, calm, and approximately 75 degrees. <br />2. A sign is posted at the entrance to the site and the boundaries are easily identifiable in the field. The <br />disturbance at this site consists of ponds, a stockpile area and a large storage area. There are multiple <br />ponds located on the site. Some of the ponds appear to have been mostly backfilled, while other <br />ponds are open. In a report related to an inspection conducted by the Division on January 28, 2000 it <br />was stated that the remainder of the ponds need to be backfilled by February, 2002. The Office of the <br />State Engineer was contacted, and there is no well permit or substitute supply plan on file with the SEO. <br />The exposed groundwater at the site it a problem cited on page three of this report. <br />3. Fuel storage tanks are located on the south side of the site along the access road. Although there is a <br />concrete berm around the tanks, there does not appear to be any type of impermeable liner installed <br />below the tanks. This issue was also cited as a problem in the 2002 inspection report (See problem 2, <br />page 3 of this report). <br />4. A large amount of equipment is being stored in the southwest corner of the site. As part of this <br />inspection, a new bond amount will be calculated for`the site. Included in the bond calculation will be <br />costs related to removal of this equipment. <br />5. Located among the equipment being stored onsite are approximately one-hundred 55-gallon drums, <br />some containing an unidentified liquid. Mr. Barbosa indicated that he thought these drums were <br />empty, but it was evident that they contained some type of liquid. In that the nature of the liquid is <br />unknown, the operator must either have the contents of the drums certified as non-hazardous or must <br />removed the drums from the site (See problem 2, page 4 of this report.). <br />As stated in this report, a new bond amount will be calculated for the site.
