(Page 4) <br />MINE ID # OR PROSPECTING ID # M-2003-003 <br />INSPECTION DATE 9/5/08 <br />INSPECTOR'S INITIALS RCO <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1. There are demolition debris, used oil lacking adequate containment, oil-contaminated <br />soil, and other non-mining-related items that are not allowed under the current approved <br />mining plan. <br />CORRECTIVE ACTION: The operator must remove and properly dispose of all these <br />items. Evidence of the completion of this removal, by letter and photos, must be <br />received at this office by the date shown at right. <br />2. There are noxious weeds present on the site, and the permit has no weed control plan <br />CORRECTIVE ACTION: The operator must contact the county weed officer to develop <br />an adequate weed control plan, and submit it for inclusion in the permit under a <br />technical revision. Submittal of the plan and technical revision must include a statement <br />committing to implement the plan promptly. This submittal must be received at this <br />office by the date shown at right.
