(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1989-06.5 <br />INSPECTION DATE: SP:ptQrnhPr 12, 2nok INSPECTOR=S INITIALS: f)R7 <br />OBSERVATIONS <br />This was a routine inspection conducted by David Bird of DRMS. Also present during the inspection was Mark <br />Heifner, consultant to Schmidt Construction Company. The site is located approximately 12 miles southeast of <br />Castle Rock in Douglas County, off of South Ridge Road. The site was last inspected 7 November 2002. <br />Weather during the inspection was wet and rainy. <br />Material mined at this site consists of rhyolite that is used to produce a variety of products from cut stone to <br />construction aggregate. The rhyolite in this area forms flat mesas covered by a thin veneer of overburden and <br />topsoil. <br />The main pit area contains several product stockpiles and some conveyors and crushers. Mining was active at <br />the time of the inspection but at a relatively slow pace with only two loaders working and no apparent activity <br />on the in-pit screens and crushers. The site is clean and maintained in good condition. Storm water controls <br />appear to be functioning properly, the hydrologic balance appears to be unimpacted, and no erosional issues <br />or noxious weed problems were observed. <br />The Division inspector inquired about an apparent fuel storage tank that was missing adequate secondary <br />containment (see attached photos). However, the Division was informed that the tank is used only for water <br />storage. <br />Mining is occurring on only one bench, and the excavation progresses in a southerly direction, with concurrent <br />reclamation of mined out areas in the northern areas of the site. Thus overburden and topsoil stockpiles are <br />small and temporary. The northern area immediately adjacent to the pit had been reclaimed the prior Spring <br />and was just now starting to show new growth. <br />A smaller pit to the west of the main pit that is used for cut stone was also inspected, but there was no activity <br />in the pit and no problems were observed. <br />The financial warranty of $375,000 currently on file is considered to be adequate and no further action is <br />required of the operator. <br />cc: t CE <br />t BL <br />I & E Contact Address F FS <br />F HW <br />NAME: Marvin Myers, Jr. r HMWMD (CH) <br />F SE <br />OPERATOR: General Shale Brick.
