ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1980---244 <br />INSPECTION DATE: -A/2£i/Q8 INSPECTORS INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site with Ms. Lisa McDonald, an <br />objector to the CCTV permit Amendment application. When the Division held an informational <br />meeting to explain our permitting process, in Victor Colorado on 6/25/08, Ms. McDonald <br />requested to accompany the Division during our routine monthly inspection. At that time <br />the Division agreed to allow her or any of the objectors to who want to accompany the <br />Division during our monthly inspection, as long as CC&V does not object. CC&V AGREED to <br />allow any objector that is willing to join the Division on our monthly inspection. Even <br />though CC&V opened the joint inspection to all those interested, only Ms McDonald was <br />present at the time of the inspection. <br />Mr. Mark Vanoni of CC&V was also present at the time of the inspection, representing CC&V. <br />Inspected the Phases I, II, III &IV leach pad areas. Explained the pumping procedures from <br />the pregnant pond to the ARD buildings. Explained that CC&V has a permit condition that <br />will not allow them to operate above the 80% total volume solution capacity for each pond. <br />How the levels for each pond were measured was explained. Ms. McDonald asked how solution <br />was transferred from the pregnant ponds to the ARD and the valley heap leach. Both the <br />Division and Mark explained the pumping procedures. At the time of the inspection the <br />Phase I pump was being maintained, even with two pumps pumping the pond levels were below <br />the 80% thresh hold. <br />The following readings were collected from the High Volume Transducers for Phase I, II, <br />III &IV pregnant ponds. <br />Phase I High Volume Transducer #1 the readout meter was not functioning # 2 as stated <br />above, the pump was being worked on, and # 3 was recorded at 47. The piezometer for the <br />pond was measured at 54 feet. <br />Phase II,III, High Volume solution collection transducer # 4 was recorded at 40.0, #5 was <br />at 37.0 and #6 was at 37.1. Piezometer for the pond was at 40.4 feet. <br />Phase IV High Volume solution collection transducer # 7 was at 42.0, # 8 was at 47.0 and <br />#9 was at 42.0 and # 10 was at 42.0 <br />Inspected the under drain area. Explained the functions of the under drain. Both the south <br />under drain and 4 inch pipes were discharging. All lower ponds were found in good repair. <br />While in the area pointed out the ground water monitoring wells for the facility and <br />explained briefly the purpose of the monitoring wells. <br />Looked at the Main Cresson mine pit and East Cresson backfill.
