Cattany <br />Division Director <br />Natural Resource Trustee <br />RE: CLG Farms, LLC; DRMS File No. M-2008-060; Receipt of 110c Construction <br />Materials Reclamation Permit Application Package <br />Dear Mr. Paynter, <br />On August 20, 2008, the Division of Reclamation, Mining and Safety received your 110c <br />Construction Materials Reclamation Permit Application Package for the CLG Farms, <br />LLC pit, File No. M-2008-060. Preliminary review of the information receh ed <br />determined that the following items must be received before the Division can consider <br />your application as being submitted and technical review can begin: <br />Application Form: The Certification Page has not been filled out completely. <br />Please submit a replacement page and include a corporate seal. If there is no <br />corporate seal than either write "no seal" or draw a seal. Please make sure that the <br />spelling of the typed names matches the spelling of the signature.In addition, the <br />Division requires proof that the applicant has the authority to sig1ron behalf of the <br />company. <br />EXHIBIT A - Location May (Rule 6.3.1(2)): A map showing N?rformation <br />sufficient to determine the location of the affected land on the ground and existing <br />and proposed roads or access routes to be used in connection with the mining <br />operation. Names of all immediately adjacent surface owners of record shall also <br />be shown. The operation location map shall be a standard 1:24,000 scale U.S. <br />Geological Survey map. The location of the proposed operation shall be shown <br />and labeled with the mine site name. <br />EXHIBIT B - Site Description (Rule 6.3.2(a)): You must address Item (a) to <br />the extent necessary to demonstrate compliance with the applicable performance <br />standard requirements of Rule 3.
