Sherman <br />Executive Director <br />Dick Wolfe, P.E. <br />Director <br />When replying, please refer to: <br />MONUMENT DAM <br />Construction File No.: C-1676A <br />Water Division 4, Dam ID: 400413 <br />Subject: Construction Acceptance of the Modifications to Monument Dam <br />Dear Mr. Kistler: <br />A final construction inspection of the modifications to Monument Dam located in Gunnison <br />County was performed by Mr. Jason Ward of our Montrose office-to confirm that the construction of this <br />project had been substantially and satisfactory completed. Our office also received "As-Constructed" <br />drawings, and construction completion documentation in accordance with Rule 10 of the State of <br />Colorado's "Rules and Regulations for Dam Safety and Dam Construction" from Barr Engineering <br />Company. Based on a review of the aforementioned documents and the results of the final construction <br />inspection, the construction of this rehabilitation project is considered to have been satisfactorily <br />completed and generally constructed in accordance with the approved plans and specifications. <br />Therefore, this construction project is accepted for full use. <br />The completion of this project did not increase or decrease the size or hazard classification of <br />this small size Significant Hazard dam. According to our records, Monument Dam has a dam height of <br />approximately 72 feet and a dam crest length of approximately 422 feet. The reservoir storage capacity <br />with the reservoir at the spillway crest elevation is approximately 467 acre-feet. <br />You are reminded that the Minnesota Reservoir and Canal Company is liable for the safety of <br />this structure and for any loss of life and/or damages caused by a failure of the dam or its appurtenant <br />structures. Acceptance of the construction by this office does not relieve the owner form this liability <br />or from any other statutory obligations. Therefore, it is in the owner's best interest to operate and <br />maintain the facility in such a manner that the safety of the dam and/or the general public is not <br />jeopardized. <br />We are enclosing a copy of Rules 12 and 15 of the Rules and Regulations for your reference <br />and use.
