Also, <br />the accompanying cover letter to the Colorado Department of Reclamation Mining and Safety <br />(the Office), which identifies all Attachments, and Exhibits associated with this Form. This <br />Form, accompanying Cover Letter, Attachments and Exhibits (less confidential items) are to be <br />available for public review. These documents "shall not be recorded but shall be retained there <br />for at least 60-days after a decision on the application by the Office and be available for <br />inspection during this period". This is required by the Colorado Department of Reclamation <br />Mining and Safety Hard Rock/ Metal Mining Rules and Regulations Subparagraph 1.6.2(1)(c). <br />"At the end of this period, the application may be reclaimed by the applicant or destroyed (Rule <br />1.6.2(2))". <br />If you would, please sign the enclosed affidavit as proof for the Colorado Department of <br />Reclamation Mining and Safety the aforementioned Form, Cover Letter, Attachments and <br />Exhibits were received by your office.
