This Amendment captures the items identified in the August 13, 2008 Mined Land <br />Reclamation Board (MLRB) hearing, as well as the May 19, 2008 letter document citing <br />a'Reason to Believe a Violation Exists, Permit No. M-1978-091-UG, Golden Wonder <br />Mine. The hearing and the letter document identified several recommendations which <br />require immediate operator/owner attention in order to abate the civil penalties imposed <br />during the hearing. In partial response to those recommendations, this letter and all <br />associated attachments represent our formal Golden Wonder Designated Mining Limited <br />Impact (11 Od) Operation Reclamation Permit Application for an Amendment. <br />This amendment is provided to add acreage (added 6.4 acres) to the existing permit area <br />(3.5 acres) necessary to address surface area needs associated with the mine site. The <br />total amount of proposed area will be 9.9 acres. Addition of this surface area is necessary <br />for implementation of the required Deadman Gulch channelization project, landslide area <br />reclamation, surface area associated with both the Level 3 and Level 6 pads for future <br />mine site operations, and the Level 3 and Level 6 access roads which are dedicated to the <br />mine site. <br />There are several components to this submittal. The required `Designated Mining <br />Limited Impact (I IOd) Operation Reclamation Permit Application Package' form is <br />contained within Attachment A. Attachment A also provides narrative responses to <br />several of the form questions, as well as maps and tables for the information requested <br />2200 E.
