INSTRUCTIONS FOR AIR POLLUTANT EMISSION NOTICE FOR NEW, EXISTING, AND MODIFIED SOURCES

<br />level under the annual consumption and output boxes as. well as in the emission estimate section. You also need to enter your best estimate of production/operation for tire. first year
<br /> of operation. An accurate estimate for the first full
<br />year of operation can allow you to avoid having to submit a revised APEN to report vour actual emissions after the first year, but may result in fees for emissions, which didn't actually
<br /> occur.
<br />This Air Pollution Emission Notice (APEN) is valid for five (5) years
<br />from the date received b the Division. A revised APEN must be
<br />submitted no later than 30 days before this expiration date.
<br />A revised APEN must be submitted whenever uncontrolled actual
<br />emissions of any pollutant not reviously re orted occurs which cxcee s:
<br />1 ton/year (icon ttainment for any pollutant 2 tons/year (attainment for
<br />all pollutants) for criteria pollutants or the appropriate reporting threshold
<br />for non-criteria reportable pollutants.
<br />A revised APEN must be submitted prior to the installation of new, or
<br />replacement of existing pollution control equipment.
<br />A revised APEN must be submitted prior to any change in the ownership
<br />or operator of a facility.
<br />A revised APEN must be submitted in order to change a permit limit or
<br />condition. Highlighted boxes for requested levels should be completed.
<br />A revised APEN must be submitted whenever a significant change in
<br />annual (calendar year) actual emissions occurs. This APEN must be
<br />submitted by April 36 of the year following the calendar year during
<br />which this change occurs. A significant change for a pollutant is defined
<br />as:
<br />Sources which emit 100 tons per year or more: any increase of 5% or
<br />50 tons/year or more of actual emissions, whichever is less.
<br />Sources which emit less than 100 tons per year: any increase of 5
<br />tons/year or more of actual emissions, except for sources of volatile
<br />organic compounds in the Metro Denver Region which must report
<br />an increase of 1 ton/year or 5% of actual emissions, whichever is
<br />greater.
<br />Sources, which emit Non-Criteria Reportable Air Pollutants: any
<br />increase of 50% or 5 tons/year of actual emissions, whichever is less.
<br />Note: For an APEN to be considered renewed all appro riate fields for
<br />current operating and company data must be filled out. The application
<br />will not be accepted if it simply notes `No Change'. See PS Memo 02-01
<br />for further information.
<br />Grouping of multiple emission points on a single APEN shall be allowed
<br />as o en as possible, provided tfie overall goals of receiving accurate and
<br />verifiable emissions information are not compromiser Please see
<br />Regulation No. 3, Part A, Section II.B.4 for guidelines regarding grouping
<br />of multiple emission points.
<br />Federal Tax I.D. No: Providing your Federal Tax I.D. no. will help
<br />the ,vision pprocess your permit application more efficiently, and
<br />will help the iJivis,on to reduce billing errors.
<br />Velocity: To the nearest i fl./min., obtained by dividing Flow Rate by
<br />area of exit.
<br />Moisture: Percent by weight of exhaust gases.
<br />Plant 1D: Note any in-house identification number for a stack.
<br />C. FUEL INFORMATION
<br />Dcscri tion of Combustion Unit: Identify the specific device that
<br />urns the ue. Axampes: team Generator #I, Crude Heater C,
<br />Rotary Kiln, Melting Furnace Beel Pulp Dryer, Asphalt Batch Rotary
<br />Dryer, Grain Dryer w/Recycle. Additional tines under this heading
<br />may be used for description, since only ONE combustion unit may be
<br />properly inventoried on an APEN forni7Fncfude manufacturer Model
<br />and Serial Number.
<br />Design Rate: Capacity of combustion unit in 106 BTU/hour.
<br />Kind of Fuel: Up to three fuels Coal, Natural Gas, #6 Residual Oil,
<br />Wood/Bark, etc.S may be listed or the combustion unit, with data on
<br />each line pertaining to that fuel.
<br />Am al Consumption: The amount of fuel used in tons of solid fuel
<br />10 - Gal of liquid hicl, or 10 set of gaseous fuel. On permit
<br />applications & revisions enter requested maximum amount. This
<br />requested maximum will become a permit limit.
<br />Heating Value: For solid fuels use BTU/LB. For liquid fuels use
<br />131 U/CIAL. I?or gaseous fucts use BTU/scf.
<br />Sulfur & Ash Content: Nearest 0.01% (sulfur), 0.1% (ash).
<br />Seasonal Use: Seasonal share boxes must total 100%.
<br />S ace I-It : The share (if any) of annual use of this fuel in this
<br />combustion unit attributed to space heating needs.
<br />To facilitate the Division's analysis, attach a sketch of the plant layout
<br />on 8 1/2"X 1 i" aper. Refer to it when helpful in your block entries.
<br />This sketch need not be exactly to scale if pertinent dimensions are
<br />annotated, including hei ht and positional distances of structures near
<br />the stacks, position of ?ie slacks relative to plant land boundaries,
<br />notable terrain features, etc.
<br />D. PROCESS INFORMATION
<br />Descri Lion of Processin Unit: identify equipment contributing. to
<br />the emissions rpm us sac or vent; use second line for descripon
<br />if needed (Only ONE processing unit per APEN form). Include
<br />manufacturer Mdel and Serial No.
<br />Raw Materials. Solvents, etc.: List all material used.
<br />Annual Consumption: List amounts of each material used annually
<br />spec, units. Several materials in a process may re. uire use of
<br />REMARKS space or additional po es. Staple these additional sheets
<br />to the APEN. On errant applications & revisions, enter requested
<br />maximum amount. This requested maximum will become a permit
<br />limit.
<br />Design Rate: Raw material capacity per hour.
<br />Finished Products: List annual production. Note both the requested
<br />maximum amount and previous calendar year estimate.
<br />estimation method. (e.g., manufacturer's data). If emission
<br />estimations are based upon methods other than those supplied by the
<br />Division or EPA, sufficient supporting documentation must be
<br />included. On permit applications revisions, enter requested nmaxi-
<br />mum amount. This requested maximum will become a permit
<br />limit.
<br />If the applicant wants the division to calculate the emissions from this
<br />source, flit box in the lower right hand corner of section E must be
<br />checked off. If applicant is requesting that the division calculate
<br />emissions, any information needed to make such calculations must be
<br />provided. Applicants for permits ntay request a copy of the division's
<br />emission estimates b , checking off the copy of prehnunmy analysis
<br />on item 13 of the application form.
<br />Be sure to fill in the year for which actual data applies space. This is
<br />the year for which the listed information is applicable.
<br />The permit will limit the source to the quantities of materials listed on
<br />this APEN. Please be sure that these quantities are accurate and will
<br />allow you to operate at the rate you desire.
<br />If a block size is inadequate for the required information, use the
<br />Remarks' space (Section A), or an additional sheet of paper.
<br />If non-criteria reportable air pollutants are emitted, please complete
<br />the addendum form also.
<br />FEES:
<br />A $152.90 filing fee is required with each APEN submitted. In
<br />addition processing fees are, charged for both initial and final
<br />a proval permits, permit modifications and transfers of ownership.
<br />Plrocessing fees will be billed at $76.45/hr based on actual (tours
<br />logged on the permit. All fees must be paid before the permit will be
<br />issued.
<br />The APEN is necessarily complex to. supply required data for
<br />technical needs for reviewing and approve permit applications. For

<br />A. GENERAL INFORMATION
<br />Normal O eration of the Source: Overall source schedule. If schedule
<br />or t is source liters significantly from facility schedule, list in
<br />remarks area.
<br />Seasonal Throu h ut: Overall process throughput (see section D.) of
<br />materials during he indicated 3-month seasons. Boxes must total
<br />100%. The Deccb entry contains throughput for January, February,
<br />and December of the DATA YEAR.
<br />B. STACK OR VENT INFORMATION
<br />l-lci ht: Vertical distance in feet from ground level to emission outlet
<br />eve .
<br />Diameter: Inside diameter of rqund exit (nearest 0.1 it). For
<br />noncircu ar exits with area A ft, use EQUIVALENT diameter:

<br />Temperature: Exhaust streanm exit temperatures under normal
<br />operating conditions, to nearest 1"F.
<br />Flow Rate: Actual cubic feet per winute at normal operating
<br />temperature and pressure, to nearest I ft huinutc.
<br />E. POLLUTION CONTROL EQUIPMENT
<br />T e of Control E ui ment: List all air pollution control equip1ment
<br />use or con to m enssions through this stack or vent, accordum to
<br />the pollutant it con?rols s (one device may be listed more than once if
<br />two or more controls are used, the FIRST encountered by the exhaust
<br />is listed as PRIMARY, then subsequent devices arc SECONDARY.
<br />Overall Collection Efficient : The combined efficiency of all control
<br />devices in removing he particular pollutant (lo O.l%).
