Please submit the fee of $216.00 to this office. <br />Please find a check enclosed for the amount of $216.00. <br />Please submit an analysis of the biosolids that are proposed to be imported and treated <br />on site. Specifically include heavy metals, pH, nitratelnitrite and any other constituents <br />that may be of concern. <br />See attached reports for representative sample analyses. <br />Per Rule 3.1.6, in the event that toxic materials are brought onto site what measures are <br />to be implemented to protect the hydrologic balance of the affected land and the <br />surrounding area? <br />Please see attached drainage study which incorporates a containment berm <br />surrounding the site with a retention/evaporation pond for on-site stormwater. <br />If the treated biosolids are to be used as a soil amendment for reclamation within the <br />permit area a revision will need to be made to the approved Reclamation Plan. Please <br />provide a detailed description of the contents of the "Class A" biosolids that will be used <br />within the permit area. Provide the recommended application rate per acre and the <br />proposed application method.
