Dear Mr. Wickstrom:
On May 19, 2008, the Division of Reclamation, Mining and Safety received a Succession of Operators application, a
Performance Warranty and a General Purpose Rider for American Home Insurance Company Corporate Surety Bond No.
in the amount of $15,985.00, for the Cortez Pit, Permit No. M-1976-059. The Division is returning these documents
for the following:
Succession of Operators Application:
The Division requires the company name listed for the successor operator throughout this document match the
company name listed with the State of Colorado Secretary of State's office (i.e. "Skanska USA Civil West Rocky
Mountain District Inc." rather than "Skanska USA Civil West Rocky Mountain District, Inc"). Please note the
omission of the comma between "District" and "Inc" and the addition of a period following "Inc". Please have
your notary make and initial this correction on page 5.
The Division requires the company name listed for the current operator throughout this document match the
company name listed with the State of Colorado Secretary of State's office (i.e. "Nielsons, Inc." rather than
"Nielson Inc.' ). Please note the addition of a comma. Please have your notary make and initial this correction on
