.. <br />• <br />Two insulated 8,200 gallon capacity storage are mounted outside the ADR on <br />concrete foundations within a bermed concrete containment structure, sized to <br />110% of the largest tank volume within containment. Tank fill status is <br />monitored from the ADR control room; the receiving tanks have alarms and <br />high-level indicators to prevent overfill; additionally, the two tanks are <br />interconnected if additional receiving volumes are required. Concrete berms <br />effectively prevent the potential mixing of cyanide solution with acid. Acid <br />storage tanks were located 30 yards from the cyanide storage area, within a <br />separate concrete containment structure. <br />One ISO container of briquettes is held in reserve pending the arrival of the next <br />shipment. Unmixed tanks are prepositioned on the dedicated concrete pad <br />before the drivers depart the site. <br />3.2 Operate unloading, storage and mixing facilities using inspections, <br />preventive maintenance and contingency plans to prevent or contain <br />releases and control and respond to worker exposures. <br />The operation is: ^ in full compliance <br />^ in substantial compliance <br />^ not in compliance ...with Standard of Practice 3.2. <br />Discuss the basis for this Finding/Deficiencies Identified: <br />• <br />All cyanide mixing and handling operations, including monitoring and <br />inspection activities, are documented as Standard Operating Procedures (SOPs), <br />which are distributed and maintained as an element of CC&V's independently <br />certified ISO 14001-based Environmental Management System (EMS). SOPs <br />detail inspections to be completed during the unloading and mixing operations. <br />These include checking showers, container seals, piping and tanks for damage or <br />leakage, and signage. The procedures also include air testing to check <br />connections are tight, and inspection of the emptied ISO-container to ensure it is <br />clean.
