• presentations and other materials have been presented in public meetings with <br />the Colorado State Senate, various county organizations, county commissioners, <br />cattlemen's associations, and other interest groups that describe the mine's <br />cyanide management practices in extensive detail. Evidence exists that <br />academic tours sometimes prompt full and open discussions of CC&V's practices <br />as examples of awell-operated heap leach facility. Summaries have also been <br />included in the 2005 corporate Report to Society published by AnglGoldAshanti <br />Ltd. The 2006 Report to Society further describes the AngloGold Ashanti s <br />commitment to the Code. <br />No offsite spills or releases have occurred in the life of the mine. Only one minor <br />cyanide-related incident has occurred (January 1997). The incident was <br />considered to be non-reportable, as no hospitalization or medical treatment <br />beyond use of emergency oxygen was required and the employee suffered no <br />adverse effects. The incident was related to a procedural error in a routine <br />maintenance action in the ADR. The cause of the incident was investigated, <br />widely discussed in routine safety meetings, and resulted in a number of process <br />equipment modifications and operational improvements. <br />Review of CC&V ERP protocols indicates that any cyanide exposure resulting in <br />hospitalization or a fatality would fall in an action category requiring the most <br />• rigorous measures to preserve human life and subsequent corrective and <br />preventive action.
