• <br />• <br />• <br />checked monthly and also sent to the supplier every six months for service. <br />CC&V are moving towards using Toxipro monitors as these units are easier to <br />use, can be calibrated to 4.7 ppm and can be calibrated in house. There are <br />presently five of these units, which are also calibrated monthly. <br />Cyanide warning signs are posted at frequent intervals around the VLF, on the <br />doors of the ADR building and on piping and vessels inside and outside the <br />ADR. Piping containing cyanide was also well marked to show flow direction. <br />Signs were clean, clear and posted in visible places. Signage instructing no <br />eating and drinking are posted on the front door of the ADR. No smoking signs <br />are located outside of the ADR. Smoking is also prohibited in the leaching, <br />processing or control room areas. <br />Eyewash/ shower units are located throughout the ADR where chemicals are <br />handled, and near the cyanide unloading station, the caustic and HCl tanks and <br />the peroxide treatment facility. The units operate at a regulated pressure of <br />about 35 psi. The units are checked daily by the process operators during the <br />pre-shift inspection and in monthly ADR Emergency Equipment inspections. <br />Fire extinguishers are primarily ABC dry units in the ADR, and only dry units <br />are located where cyanide is handled. These are checked monthly during ADR <br />Emergency Equipment inspections and were observed to be easily accessible and <br />were clearly tagged. <br />Electronic copies of all MSDSs are accessible to all employees via the company <br />Intranet or Carlton Security. First Aid procedures for cyanide are also posted at <br />strategic locations in and near the ADR. <br />CC&V's incident reporting and investigation procedure is also used for reporting <br />and investigating near misses. Incidents are investigated by the supervisor and <br />reviewed by management.
