~~~_\ <br />SMITH WILLIAMS CONSULTANTS, INC. <br />7.0 Phase IV Liner Re-Certification <br />During Phase IV VLF construction, there were two areas within the VLF that were excluded <br />from certification: 1) the Load-Out Bin (LOB) platform, and 2) a portion of the Phase IV haulage <br />ramp. These areas were excluded from the Phase IV certification because the thickness of DCF <br />over the liner did not fully meet the project specifications for haul truck trafficking. <br />As part of the Phase 5 VLF, these areas will be uncovered, inspected, repaired (if needed), and <br />certified. It is important to note that this work will be conducted in accordance with the <br />Construction Quality Assurance (CQA) plans and specifications presented in Appendix H. All -- <br />activities will be monitored and documented by a qualified CQA monitor. The exposed liner and <br />seams will be visually inspected and non-destructively tested, as needed, to assess the integrity of <br />the geomembrane. Any damage geomembrane will be repaired per the project specifications <br />(Appendix H), by qualified personnel. Any repairs and seaming will be tested to verify liner and <br />seam integrity. <br />Upon completion of the work, a certification report will be prepared and submitted to CC&V. <br />The certification report will include the following: <br />^ Drawings showing the limits of the certification. These drawings will also include <br />the location of liner tests, liner defects, and repairs <br />^ CQA monitoring reports for removal of the DCF, geomembrane inspections and tests, <br />and liner repairs <br />^ Liner acceptance-forms after repairs have been completed and verif ed <br />^ CQA monitoring reports for re-placement of DCF cover over the liner. <br />The certification report will be issued and signed by a Colorado Registered Professional <br />Engineer. <br />• <br />S:\PROJECTS\1125 CC&V PHASE 5 VLF1H2 - DESIGNWREQUA VLF\VLF PHASE 5 FINAL REPORT V2.DOC 4'~ <br />
