n <br />Item 5 - Identify thf: receiving water. Receiving waters are any waters which are contained in or flow in or through the state of Colorado (except <br />water withdrawn for use until use and treahnent have Ueen corpleted). This definition. includes all water courses, even if they are usuall <br />dry. <br />Item 6 - Self explanatory. <br />Item 7 - Indicate whether or not your facility has or is subject to the following environmental permits (not including the one now being applied fc <br />a) Irdicate your Mined Land Reclamation Board permit number in item 7.a. The anniversary date of your mining permit issuance by d <br />Colorado Mined Land Reclamation board is also required. <br />b) ]f yon rurrenlly have a permit from the Division for the discharge of stormwater, include the nwnber in item 7.f. <br />Item 8 - This application covers the discharge to receiving waters of two types of effluent: process water and stormwater. It is anticipated that, <br />generally, all nomnetallic minerals production operations will come under the stormwater portion of the application, while only some of <br />operations will also have a discharge of process water. <br />Process water discharges: Submittal of this application is required if you discharge any process waters. Process generated wastewater <br />include: product wash water, maintenance/equipment wash waters, transport waters, scrubber waters (crushers or classifiers), mine <br />dewatering (groundwater and/or runoff), and stornwater runoff which mixes with process generated wastewater prior to sampling. If yo <br />discharge includes any of these sources, you must complete items 9 -19 of the application as well. <br />Il'you answered /1~o~to all tl~e questions, please skip to item 18. <br />stormwater discharges: Submittal of this application is required from nonmetallic minerals production operations if there is a possibil <br />of discharge of stormwater that can come into contact with any overburden, raw material, intermediate products, finished products, <br />byproducts or waste products located at [he site of snch operation, except as specified above under process water. Por example, runoff <br />haul roads, ex)uipment storage areas, or shipping and receiving areas world be considered a stormwater discharge. <br />Item 9 - This map is indicated to serve as an area map attachment to the permit. A legible submittal is reyuired. The map should be on paper 8 <br />11 inches. <br />Item 10 - This is a facility sketch for inclusion in the permit. A legible submittal is regtured. The sketch should be on paper 8 2 x 11 inches. <br />Item 11 - If you answE;red YES to any of these questions, include an explanation of the situation. For example, explain why yott suspect or know <br />groundwater contamination, and what has been done to remedy the situation. include any applicable analytical data. <br />item 12, 17-Self explanatory. <br />Item 18 - This reyuirement applies to all facilities. Completion of a Stormwater Management Plat (SWMP) is required. (Submittal of the ac <br />plan is not required.) The permit application shall include a signed certification that the SWMP has been completed. For a detailed list c <br />SWMP requirements, see Appendix B of the application. <br />Item 19 - The application mast be signed by both the operator and owner to be considered complete. The person who is applying for the pernrii <br />owner or operator, as indicated in item I) and who silms the application will be the legally responsible party. In all cases, it shall be sign• <br />follows: <br />a) In the case of corporations, by a principal executive officer of at least the level ofvice-president or his or het duly authorized <br />represenl:ative, if such representative is responsible for the over-all operation of the facility from which the discharge described in the <br />application originates. <br />b) In the case of a partnership, by a general partner. <br />c) In the case of a sole proprietorship, by the proprietor. <br />d) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking elected official, or other duly <br />authorized employee. <br />
