Cattany <br />Division Director <br />Natural Resource Trustee <br />Re: Brent Pit Conversion, M-1978-072, Receipt of 112 Construction Materials Reclamation Permit <br />Application Package j,,, co v- Q l 0, k e-- <br />Dear Mr. Morrison, <br />On July 24th, 2008, the Division of Reclamation, Mining and Safety received your 112 Construction Materials <br />Reclamation Permit Application package for the Brent Pit, M-1978-072. Preliminary review of the information received <br />determined that the following items must be received before the Division can consider your application as being <br />submitted ("filed") and technical review can begin: <br />APPLICATION FORM: On page two of the application, item #6 was not filled out. Please re-submit three <br />copies of page two with all the required information filled out. <br />Page four of the application was not submitted, thus items #12-16 were omitted. Please submit three copies of <br />page four of the application packet with the required information filled in. <br />Also, on page eight of the application the date on the notary line was not filled in. This date must match the date <br />above the "Applicant/Operator or Company Name" line. Please re-submit one original and two copies of page <br />eight with all the required information, signatures and seals. <br />Enclosed is a copy of pages two, four eight of the application. <br />EXHIBIT Q - Proof of Mailing of Notices to County Commissioners and Soil Conservation District (Rule <br />6.4.17): Please submit proof of actual delivery or proof of mailing by Certified Mail, return receipt requested, of <br />Notice of the Application to the Board of Supervisors of the Soil Conservation District, if the mining <br />operation is within the boundaries of a Soil Conservation District. <br />EXHIBIT R - Proof of Filing with County Clerk and Recorder (Rule 6.4.18): You must submit an affidavit <br />or receipt indicating the date on which the application was placed with the local County Clerk and Recorder. <br />Newspaper Publication and Public Notice: Within your application exhibits you submitted a Fax transmission <br />to the local newspaper requesting that they publish the public notice document that you submitted for four weeks. <br />Unfortunately, according to rule 1.6.2, this publication must be made after your application is considered <br />"filed/submitted" with the Division.
