ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-9004-0 1.3 <br />INSPECTION DATE: A.197 4A INSPECTOROS INITIALS: _BK_ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site as part of sites to be <br />inspected during the fiscal years 2007-2008. <br />1) Site is a phased 112 sand and grave operation located approximately 10 miles east of <br />Pueblo. <br />2) Total permit area is 364,maximum area to be affected at any given time is 25 acres. <br />3) Present during the inspection was Mr. Henry Rivera, division manger for Trans <br />Colorado Concrete. Site was not active at the time of the inspection. According to <br />Mr. Rivera and what is reported in the annual report, ASIDE FROM REMOVAL OF material <br />from existing processed piles,no active mining had taken place on site for the past <br />two years due to lack of demand. <br />4) Mining operation requires dewatering the pit through an NPDES PERMIT when active. <br />5) Material is screened and washed. There are two settling ponds and two fresh water <br />ponds on site. Given the mines close proximity to the Arkansas river, all ponds were <br />holding water. The settling pond, in addition to holding water was also vegetated <br />with wet land vegetation at the time of the inspection. <br />6) The entire permit area was driven and inspected, permit area is fenced. Large <br />stockpiled processed material was noted along the East side of the permit area. <br />7) Along the North and West side of the permit area, along Nyberg road and State <br />highway 50 there is a berm that acts as a view shed. Berm was found to be in good <br />repair. <br />8) The operator also has in place four GW monitoring wells for static water table, to <br />make sure wells located with in 600 feet of the permit area are not impacted due to <br />the dewatering operation. <br />9) The current warranty for the site is $410,693.00 for a maximum disturbance of 25 <br />acres. Operator is aware prior to disturbing additional acres, a written approval <br />from the Division has to be obtained. <br />10)Permit sign was properly posted at the access to the property.
