Bid Award <br /> The apparent low bidder must sign and submit the attached Bid Schedule, and a certificate of <br /> insurance with the following limits: <br /> 1. Standard Workers Compensation and Employer Liability, as required by State Statute <br /> including occupational disease, covering all employees at the work site.. <br /> 2. General liability (Public Liability and Property Damage) (Minimum): <br /> a. Combined single limit of$600,000 written on an occurrence basis; <br /> b. Any aggregate will not be less than $1 million; <br /> c. The CONTRACTOR must purchase additional insurance if claims reduce the annual <br /> aggregate below$600,000; <br /> d. The State of Colorado must be named as additional insured on each comprehensive <br /> general liability policy; <br /> e. Certificates of insurance must be provided to the state within ten (10) working days after <br /> receipt of award; and, <br /> f. Insurance shall include provisions providing notification to the State ten (10) days prior to <br /> cancellation of the coverage. <br /> 3. Automobile Liability (Minimum). The CONTRACTOR must carry a minimum of $600,000 <br /> combine single limit. <br /> VENDOR OFFSET <br /> Pursuant to C.R.S. 24-30-202.4 (as amended), the State Controller may withhold debts owed to <br /> state agencies under the vendor offset intercept system for: (a) unpaid child support debt or <br /> child support arrearages; (b) unpaid balance of tax, accrued interest, or other charges specified <br /> in Article 22, Title 39, C.R.S.; (c) unpaid loans due to the student loan division of the <br /> Department of Higher Education; (d) owed amounts required to be paid to the unemployment <br /> compensation fund; and (e) other unpaid debts owing to the State or any agency thereof, the <br /> amount of which is found to be owing as a result of final agency determination or reduced to <br /> judgment as certified by the controller. <br /> Project Work Description <br /> The General Conditions for Purchase Orders and Purchase Order Terms and Conditions apply <br /> to all work. <br /> Site Description <br /> The purpose of the Project is to backfill a subsidence feature located on the north facing flank of <br /> the Grand Hogback south of New Castle, Colorado. The feature is located approximately 1,000 <br /> feet south of the frontage road south of the Colorado River and Interstate Highway 70, and <br /> about 420 feet above the elevation of the frontage road. See the attached Map. The Grand <br /> Hogback is very steep in this location. Because of the steepness of the slope, access to the <br /> feature is limited to foot traffic, possibly pack animal and by helicopter. Note, however, the site <br /> is not conducive to landing a helicopter in the vicinity of the subsidence feature. Tracked <br /> machinery cannot access the subsidence feature. It is likely that a staging area can be <br /> established within 800 feet (measured horizontally) from the subsidence feature at the base of <br /> the Hogback, near the frontage road. <br /> 2 <br />
