The contract will be signed on March 17, 2000 at <br /> 4:00 p.m. To complete the process of bid award, the following information must be received by me in the <br /> offices of the Division of Minerals and Geology on or before the date and time set for signing: <br /> 1) Five executed copies of the enclosed agreement, dated on the signing date. <br /> 2) A certificate of insurance and Additional Insured Endorsement, in the amounts given in <br /> Article 3, INSURANCE, of the General Conditions. The certificate of insurance must <br /> show the State of Colorado as an additional insured. <br /> 3) A performance bond and a labor and materials payment bond in the amounts of 100 <br /> percent and 100 percent respectively of the bid amount($19, 944.00). The bonds must <br /> be submitted using the State forms and must be dated the same day as the Agreement <br /> (the contract signing date). Copies are enclosed. <br /> 4) A project schedule, as specified in Article 5 PROJECT SCHEDULE of the General <br /> Conditions. <br /> 5) A completed AML Contractor Ownership and Control Information Package. <br /> When the above documents have been submitted and found to be in order, and the contract agreement <br /> fully-executed, the Notice to Proceed will be given.
