WW-1 Revision No. 1 Replaces Procedure: NIA <br />Page: 2 of 11 Revision Date: 03/09/08 Dated: N/A <br />SECTION 1: SURFACE FACILITY EMERGENCY RESPONSE PROCEDURES <br />Preface <br />These procedures have been developed for use by Energy Fuels Resources Corporation (EFR) personnel <br />during any response to an emergency situation at the Whirlwind Mine site surface facilities. An emergency <br />situation can include, but is not limited to, spills of hazardous materials (HazMat), fires, accidents involving <br />personnel and/or material transport, or any combination of the above. Note that all EFR personnel located <br />on. the surface during an emergency situation are, required to respond to the emergency. <br />These procedures are not to be used for responding to alarms associated with routine operational problems <br />that occur within the site, process and monitoring systems, unless those problems could result in an <br />emergency situation. Examples of routine operational problems include intrusion 'alarms and routine <br />process alarms at the wastewater treatment facilities. <br />Any emergency situation will be successfully resolved by a phased response consisting of notification, <br />operations, and remediation. These three phases will be implemented in concert or sequentially depending <br />upon the specific situation and available personnel. <br />PHASE TWO: OPERATIONS <br />PHASE THREE: RENIEDIATION <br />The first person to arrive at the location of an emergency situation becomes the First Responder to <br />the incident and assumes responsibility for the subsequent emergency response until they are <br />relieved by an Emergency Coordinator, or voluntarily relinquish their authority to a more qualified <br />person. The First Responder can be any Energy Fuels employee or a contractor's employee (e.g. <br />truck driver). <br />PHASE ONE: NOTIFICATION <br />PHASE 1.0 NOTIFICATION <br />The PRIMARY RESPONSIBILITY of the First Responder in an emergency situation is the prompt <br />NOTIFICATION of other site personnel. The First Responder shall immediately relay an incident evaluation <br />to at least one other person prior to initiating the operations phase during an emergency response. The <br />incident evaluation must include the following information, at a minimum. <br />a) The location of the incident. <br />b) The nature of the incident. <br />c) The extent of injury, if applicable. <br />d) The type of material spilled, if known and applicable. <br />e) The physical extent of the spill area, if applicable. <br />f) The First Responder's intended course of action. <br />g) Available communication devices, if communication must be maintained. <br />ENERGY FUELS RESOURCES HEALTH & SAFETY PROCEDURES
