• <br />C <br />:• <br />Plan has been included that addresses containment and treatment of mine water in <br />detail.The majority of the surface facilities for the operation are located in Colorado; <br />however, six of the seven proposed new ventilation shafts are located in Grand County, <br />Utah. Installation and reclamation of these ventilation shafts will be addressed in a Notice <br />of Intent to Commence Small Mining Operations (i.e., five acres or less surface <br />disturbance) with the Utah Division of Oil, Gas and Mining. One existing ventilation shaft <br />(Ten Straight) for the Packrat Mine and one proposed new ventilation shaft are located on <br />the Colorado side of the state line and have been included in this permit application. It is <br />anticipated that reclamation bonds for the project will be held by both DRMS and DOGM <br />in accordance with agreements in place between BLM and these state agencies. <br />The permittee for this site is Energy Fuels Resources Corporation. The name has been <br />shortened throughout this application to Energy Fuels or EFRC. <br />Whirlwind Mine 07 (rev. April 08) <br />i-4 <br />
