Governor <br /> 23478 Highway 12 Greg E.walcher <br /> Trinidad, Colorado 81082 Executive Director <br /> Ntichael B.Long <br /> Re: Notice of Award, Trinidad Basin Mine Project, Bid Number PKA-9-440 Division Director <br /> Dear Sir or Madam: <br /> This letter is to inform you that your company is the apparent low bidder on Project PKA-9-440, the Trinidad <br /> Basin Mine Project. The contract will be signed on July 23, 1999 at 4:00 p.m. To complete the process of bid <br /> award, the following information must be received by me in the offices of the Division of Minerals and Geology <br /> on or before the date and time set for signing: <br /> 1) Five executed copies of the enclosed agreement, dated on the signing date. <br /> 2) A certificate of insurance and Additional Insured Endorsement, in the amounts given in Article <br /> 3, INSURANCE, of the General Conditions. The certificate of insurance must show the State <br /> of Colorado as an additional insured. <br /> 3) A performance bond and a labor and materials payment bond in the amounts of 100 percent <br /> and 100 percent respectively of the bid amount($37, 044.44). The bonds must be submitted <br /> using the State forms and must be dated the same day as the Agreement(the contract signing <br /> date). Copies are enclosed. <br /> 4) A project schedule, as specified in Article 5 PROJECT SCHEDULE of the General Conditions. <br /> 5) A completed AML Contractor Ownership and Control Information Package. <br /> 6) A completed Procurement Registry form. <br /> When the above documents have been submitted and found to be in order, and the contract agreement fully- <br /> executed, the Notice to Proceed will be given.
