Ms. Maggie Van Cleaf-Division of Minerals and Geology <br /> Peacock Mine Access Road Recontour and Vegetation <br /> May 14, 1996 <br /> Page 2 <br /> HYDRAULIC EXCAVATOR, HOURS <br /> ERM EnviroClean will provide a CAT322L/215C hydraulic tracked <br /> excavator with a 30-inch bucket. The cost of mobilization, transport, labor, <br /> fuel, routine maintenance and local per diem for the operator are included in <br /> the hourly rate of$392.45. If the 40 hours were guaranteed, the hourly rate I I <br /> would be $242.45. <br /> REVEGETATION ERM <br /> ERM EnviroClean will provide labor, material and equipment to apply State <br /> Supplied Seed and ERM EnviroClean supplied mulch as specified in the IFB. <br /> The lump sum cost for the revegetation is$4,765.00. <br /> Health and Safety <br /> ERM EnviroClean employees participate in a corporate: health and safety <br /> program that requires regular training, medical monitoring, and if required, <br /> the use of site-specific health and safety plans. ERM EnviroClean personnel <br /> have proven experience in the development and implementation of health and <br /> safety plans. <br /> Corporate Insurance and Bonding <br /> ERM EnviroClean maintains insurance coverage that meets the requirements <br /> specified in the Invitation For Bid(IFB). The sample Certificate of Insurance <br /> illustrates the coverages in-place and the limits of liability. ERM <br /> EnviroClean also has extensive Contractor's Pollution Liability coverages not <br /> generally acquired and maintained by most environmental restoration <br /> companies. A overview of ERM EnviroClean's insurance and bonding <br /> coverages is presented as part of this IFB response. <br /> Cost and Project Personnel <br /> ERM EnviroClean can perform the offered scope of work in the time frame <br /> presented in the IFB, i.e. May 20 through May 24, 1996. Mr. David Hughes <br /> of ERM EnviroClean has been selected to be the on-site equipment <br /> operator/supervisor and shall report to the Division of Minerals and <br /> Geology's Project Manager for daily direction <br /> ERM EnviroClean will allocate additional resources to the project if <br /> necessary. Both Mr. Dan Hinds of ERM EnviroClean and Mr. Don Malotte <br /> of ERM-Rocky Mountain, Inc. will be available to provide project support as <br /> requested by the Divisions Project Manager. <br /> Recycled Nper <br /> 1� <br />
