ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-19Rn-944 <br />INSPECTION DATE: 6127/08 INSPECTORS INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site as a result of a phone call <br />from Tim Commer of CC&V, regarding an incident to the barren header pipe by a dozer <br />operator. <br />On 6/26/08, Mr.Commer left a message in my voice mail stating that a dozer operator had <br />damaged the barren header pipe and allowed solution in the double lined area. Even though <br />no solution left the double lined area, the Division had requested to be informed about <br />any impact to any solution delivery system that causes delivery of any solution to areas <br />not designed be reported to Division. <br />Met with Tim in Cripple Creek late afternoon on 6/26/08 and he informed me that a dozer <br />operator accidentally damaged the barren header pipe along the North side of the Phase I <br />leach pad and allowed solution to spread along the double lined area, where it was not <br />designed to flow. No solution ever left the double lined area. He also stated that once <br />the pressure drop was noted in the process plant, as designed the system shut down and the <br />repair work on the steel pipe was completed and system was functioning as designed. <br />He was informed that the area will be inspected the next day. <br />On 6/27/08, met with Tim on site and looked at the are of impact. As reported by Tim, the <br />barren header pipe where the damage had occurred had been repaired and the pipe was <br />operating as designed. There was evidence of the over flow solution from the pipe. The <br />welded portion of the pipe was still exposed and not covered with ore at the time of the <br />inspection, A few pictures of the area were taken, to be included with the inspection <br />report. No solution ever left the double lined area. However, even though no solution left <br />the double lined area, the fact that a dozer operator could not clearly tell where the <br />header pipe was during the routine maintenance of the pad will be sited as a possible <br />problem and DRMS, request CC&V, the additional measures that they have implemented so <br />occurrences like that will not take place again. At present there are clearly defined <br />markers letting any one know of buried liner and pipes. CC&V will need to tighten <br />procedures when large machinery is operating with in buried lines. Both Tim and Kevin <br />agreed that will be implemented to avoid similar incidents in the future. <br />Once the mine Amendment # 9 is approved, this portion of the pad is proposed to be <br />removed.
