(Page 2)
MINE ID # OR PROSPECTING ID #: M-200$11111-
INSPECTION DATE: -a1 .J14/ng INSPECTOR'S INITIALS: _JKB.P
OBSERVATIONS
r
1. This inspection was conducted as a pre-operational inspection related to an amendment application
for a 1 12c permit, as well a monitoring inspection as part of the Division of Reclamation, Mining, and
Safety's four-year monitoring program. Present at the inspection were Kate Pickford, DRMS, and Ward
Priestly, . Weather at the time of the inspection was clear, calm, and approximately 60 degrees.
i
2. The application proposes to add 30 acres to the existing permit. The area to be added to the permit
contains primarily native grasses.. It previously contained some pinyon and juniper, but these have be
destroyed by a fire that occurred sometime in the past. The only structures near the proposed I
disturbance are a fence and utility poles. The current operation is already mining around the poles in
the permit area. The same procedure will be used in the amended area.
3. The remainder of the operation appears to be operating according to the approved plan. The
processing facilities are contained within the excavated area. There are large stockpiles of processed
materials, and topsoil has been stockpiled and preserved.
4. Current operations at the site appear to fall within the parameters of the approved permit. There does
appear to be any problems that would prevent approval of the proposed amendment.
I & E Contact Address Cc: ? CE
? BL
NAME: Dr. Michael A. DeJohn ? FS
?
