CONDITIONS FOR APPROVAL
^ The proposed operation does not anticipate exposing groundwater. Therefore, exposure of ground water must not
occur during or after mining operations.
® The proposed operation will consume ground water by ®evaporation, ^dust control, ^reclamation, ®water
removed in the mined product, ^processing. Permit No. 67265-F has been issued fora 30.0-acre gravel pit
pond for said uses,,with the total appropriation amount limited to 75.7 acre-feet per year.
® Prior to approving a well permit, the applicant must conduct a field inspection of the site and document the
locations of all wells within 600 feet mile of the permit area. The applicant must then obtain a waiver of
objection from all well owners with wells within 600 feet of the permit area or contact this office regarding the
600 foot spacing notification hearing process. The only well within 600 feet of the permit area is owned by
the applicant.
® The proposed operation may have potential impact on existing water rights near the project location. Please
provide evidence that the proposed project will not alter or impact the historic operation of existing water rights.
Any discharges due to dewatering must be returned to the Yampa River such that no material injury will
occur to existing water rights.
® The applicant has indicated plans to use other, offsite water for uses at the permit site (i.e. dust control, site
operations, etc.). The applicant will need to provide evidence to this office that the water is permitted or decreed
for such uses.

COMMENTS: As noted in Condition 10 on Permit No. 67265-F, pursuant to Rule 17.1.4 of the Water Well
Construction Rules, the owner shall submit, after initiation of construction, site plan and cross section drawings showing
the extent of the intended excavation, the maximum depth of the pit, the initial static water level and the initial date of
ground water exposure in the pit.
