PART III <br /> Page -12A of 13 <br /> PERMIT NO: CO-0000396 <br /> A. SPECIAL REQUIREMENTS - ACTIVITY PERMIT <br /> 1 . The permittee shall submit a written request 90 days prior to discharge <br /> to the Colorado Water Quality Control Division and the U.S. Environmental <br /> Protection Agency for any change in discharge points. The request shall <br /> be accompanied by area maps which shall meet the specifications set <br /> forth below. Each discharge point shall be numbered consecutively <br /> (001 , 002, 003, etc. ) and designated on the map. The Division shall <br /> approve or deny proposed discharge points in writing, or may in its <br /> discretion require that the permittee accept different effluent <br /> limitations as a condition of approval or submit a new permit application. <br /> Approval of additional discharge points shall not consist of permit <br /> modification, and no public notice or hearing will be required. <br /> 2. The permittee shall submit a written request to the Colorado Water <br /> Quality Control Division and the U.S. Environmental Protection Agency <br /> to deactivate a discharge point. The Division will grant or deny the <br /> request. in writing. If the request is granted, monitoring and reporting will <br /> not be required for such points. Any discharge from deactivated points <br /> shall be considered a violation of the permit. <br /> 3. Area maps shall be made from USGS topographical maps (7.5 or 15-minute <br /> series) or other appropriate sources as approved by the Colorado Water <br /> • Quality Control Division. Each revised area map shall be 81 inches by <br /> 11 inches and reproducible by rapid printing methods or as approved by <br /> the Division. Where additional 81-inch by 11-inch maps are required <br /> to show the area of operation, they shall be numbered and a key shall be <br /> shown on the first map. The first map section shall have the company <br /> name, mine/job name, address, and NPDES number clearly printed thereon. <br /> Also, one line of latitude and one line of longitude shall be marked on each <br /> map section. The area map(s) shall delineate the following , using the <br /> graphics as indicated: <br /> (a) Existing Area of Operation (Solid Outline) <br /> (b) Existing point source (Solid Triangle) <br /> (c) The projected area of operation for <br /> the 5-year life of an NPDES permit ------------ (Dashed Outline) <br /> (d) Projected point source for the 5-year \ <br /> life of an NPDES permit (Opened .Triangle) <br /> Amended <br /> 12 <br />
