Long <br />Division Director <br />Subject: Permit C-84-063, Renewal Number 3, AVS Check <br />Dear Mr. Self- <br />During a pre-issuance Applicant Violation System (AVS) check for Renewal 3, four discrepancies <br />were found between the AVS database and the interest information supplied during the renewal <br />process. Documentation is required to make changes to the AVS database. Documentation means <br />certified copies of letters of resignation or appointment, minutes of board meetings, corporate <br />records, -etc. (as are filed in Appendix O). <br />For Western Association Energy Corporation: <br />The AVS database includes Robert L. G. Watson as CEO, as well as President and Director, with <br />a begin date of 4/1/94. A permit page of 1995 listed Mr. Watson as CEO. The new Identification <br />of Interest dated 2/8/00) does not indicate this appointment. If Mr. Watson is the CEO of Western ( lease provide a revised 9A (change title to read it/ <br />Association Energy Corporation, p page President/CEO). If he is not the CEO, please provide documentation to change the AVS database. <br />The AVS database includes Chris Williford as an Assistant Secretary and Treasurer with begin <br />dates of 4/1/94 in addition to his listing as a Vice President. A 1995 permit page also listed Mr. <br />Williford as an Assistant Secretary and Treasurer. If Mr. Williford is an Assistant Secretary and <br />Treasurer, please provide a revised page 9A (change title to read Vice President/Assistant <br />Secretary/Treasurer). If he is not an Assistant Secretary or the Treasurer, please provide <br />documentation to change the AVS database. <br />Abraxas Petroleum: <br />The AVS database includes Joseph Kilchrist as a Vice President with a begin date of 4/94. A 1995 <br />page of the permit lists Mr. Kilchrist as a Vice-President. Mr. Kilchrist does not appear in the list O <br />of current officers (pages 9B or 9C), does not appear in the history (page 9D), nor is there any J <br />documentation in Appendix O. Was Joseph Kilchrist ever an officer of Abraxas? If not, please <br />advise. If so, please provide documentation or a revised page as may be appropriate. <br />STATE OF COLORADO <br />:l <br />,a
