August 10, 2007 <br />Quality Assurance Plan Version 12 <br />SOPAD018.08.07.12 Page 28 of 96 <br />7 SAMPLE CUSTODY & SAMPLE HANDLING <br />Sample custody begins with the receipt of sample containers from the client and continues beyond preparation <br />and analysis to the proper disposal of primary and secondary sub-samples. Complete and accurate <br />documentation must be provided at all stages of custody. There are many key elements to sample custody <br />including laboratory security, chain of custody records, sample storage, internal custody logs, sample tracking <br />within the laboratory, control of subcontracted work, and sample disposal. <br />7.1 Laboratory Security <br />A secure facility is essential to maintaining sample and data integrity and to providing safety to employees and <br />visitors. ACZ has an electronic security system based on anti-pass back protocols, which controls and limits <br />access to only authorized personnel. The following steps have been taken to ensure this security: <br />• All entryways are armed and a proximity reader at the east entrance and west shipping entrance <br />allows access to an employee only after he/she passes their card. <br />• Employees may enter/exit only through the west door at Log-In and the east door next to the , <br />lunchroom. <br />• During normal business hours, public access into the building can be made at the front entrance , <br />and the west shipping entrance. Both doors are equipped with a buzzer. <br />• The outside doors at the west shipping entrance remain unlocked; however, the doors between the <br />vestibule area and sample receiving area are equipped with an anti-pass back system. <br />• Building access is limited to specific hours of the employee's shifts. <br />• All employees are required to use their access cards to enter and exit the building. , <br />• If any employee does not have their access card, they must notify ACZ's CFO as soon as possible <br />and must sign in and out during the day using the visitor's register at the front desk. This <br />ensures a record is maintained of which personnel were in the building at any time. <br />• If employees fail to use their card, the anti-pass back system denies access for that card the next <br />time it is used. The employee must report to ACZ's CFO if this occurs. <br />• Visitors must enter and exit through the main entrance and must sign the register at the front desk <br />upon arrival and before departure. <br />• Lab personnel must escort visitors as long as they remain on the premises. I <br />• Emergency Exit doors are to be used only for emergency purposes. If a door is opened, a siren <br />alarm will sound. <br />• It is against company security policy to loan or transfer access cards to anyone, including other <br />ACZ employees.
